Health Record Requirements
New York State Public Health Law 2165 requires post-secondary students born on or after January 1, 1957 to provide proof of immunization for measles, mumps, and rubella prior to registration and attending classes. All full- and part-time students are required to provide proof of immunization prior to registering for classes. A student who fails to submit proof of immunization by the end of the first semester will not be allowed to register again until such proof is submitted to the College Health Services.
All nursing students who register for a practicum/clinical course must submit a completed health history and physical examination form. Any student who fails to complete this requirement is not permitted to register for the practicum/clinical course.
Information regarding these requirements can be obtained at the College’s Health Center by calling (718) 405-3472. New York State Public Health Law 2167 pertains to all college students enrolled for six or more credits per semester.
The College of Mount Saint Vincent is required to maintain the following for each student:
- A response to receipt of meningococcal disease and vaccine information signed by the student or student’s parent or guardian. This must include information on the availability and cost of meningococcal meningitis vaccine (Menomune),
and either
- A record of meningococcal meningitis immunization within the past ten years, OR
- An acknowledgement of meningococcal disease risks and refusal of meningococcal meningitis immunization signed by the student or student’s parent or guardian.
